Bulk Billing vs Gap Fee: What’s the Difference When Seeing a Psychologist?

When you’re ready to book an appointment with a psychologist under a Mental Health Care Plan, one of the first questions people ask is:

“Is this bulk billed, or do I have to pay a gap?”

Let’s break down what that means — and what to expect financially when accessing psychological support in Australia.


What is Bulk Billing?

A bulk billed service means you don’t pay anything out of pocket. The psychologist accepts the Medicare rebate as full payment for the session and bills Medicare directly.

In other words: you swipe your Medicare card, and the cost is covered — no extra fees.

✅ Pros:

  • No out-of-pocket costs
  • Accessible if you’re on a low income, concession card, or struggling financially

⚠️ Things to keep in mind:

  • Not all psychologists offer bulk billing
  • Availability may be limited
  • Often only applies to weekday hours or certain demographics (e.g. students, pensioners)

What is a Gap Fee?

A gap fee (or out-of-pocket cost) is the difference between the psychologist’s fee and the Medicare rebate.

With a valid referral and Mental Health Care Plan, you’re entitled to a Medicare rebate of $96.65 per session with a general registered psychologist. (This increases to $137.05 for sessions with a clinical psychologist.)

The psychologist sets their own fee, and you pay the “gap” between their full fee and your rebate.

  • Psychologist charges $180
  • Medicare rebate: $96.65
  • You pay: $83.35 out of pocket

✅ Pros:

  • Wider range of experienced clinicians to choose from
  • More flexibility in session times and appointment types (e.g. after-hours, longer sessions)

⚠️ Things to keep in mind:

  • You pay the full amount upfront and claim the rebate after (or the practice processes it for you)
  • There may be wait times for more popular practitioners

Which Option Is Best for You?

Both bulk billing and gap fee services can provide excellent care — it depends on your situation and preferences.

Consider the following:

  • Do you need low- or no-cost options right now? → Try bulk billing
  • Are you after a specific psychologist or more availability? → Gap fee may give you more options
  • Are you eligible for concession-based bulk billing? (E.g. health care card, student, pensioner)
